Ready to take control of your health?
Looking for a health care practitioner to help you on your journey?

You have come to the right place!

Thank you for considering Foothills Functional Medicine to assist you on your journey to achieve optimal health. We take great pride in working with our patients to create a personalized approach to their health care.

What We Offer

Our goal is to help patients get to the root cause of their illness and achieve optimal health and wellness. We do not provide traditional primary care services. We encourage patients to maintain a relationship with their Primary Care Provider (PCP).

Our office is not contracted with any insurance companies. However, we can provide you with a superbill to submit to your own insurance company for reimbursement. We do accept HSA/FSA.

Initial visits are typically 60 minutes, and maybe longer depending on your health concerns. Cost for follow ups will depend on the length of time needed for the appointment. Our providers spend additional time prior to your visit reviewing your questionnaires and any medical records prior to your initial visit.

Frequently Asked Questions

  • How do I schedule an appointment?

Call our office at 208-888-6886 or send us a message through our Contact page, and our team will get you scheduled. If we miss your call, we’ll return it as soon as possible.

  • What should I do before my first visit?

Please complete your new patient intake paperwork at least 5 days before your appointment. At a minimum, please include:

    • Medical History Questionnaire
    • Medical Symptom Questionnaire
    • Social and family history
    • Current medications and supplements

Depending on the reason for your visit, you may be asked to complete additional forms. Please also share any recent lab work or procedures relevant to your visit. You can email forms to info@fmidaho.com or fax them to 208-938-0355.

  • How long is the first visit?

Your first visit may take 60–90 minutes, depending on the complexity of your health concerns. Your provider will take a comprehensive health history and review any previous lab testing or records. From there, your provider may recommend specialty testing, medications, nutritional supplements, specific diets, or referrals based on your history and needs.

  • What happens at follow-up visits?

At follow-up visits, your provider will review the results of any labs or specialized testing you’ve completed. Depending on your needs, your provider may prescribe medications, hormones, supplements, lifestyle changes, or a nutrition plan. How many visits you need depends on your individual goals. The aim is for you to reach your wellness goals and need only occasional visits. If you’re receiving hormone therapy or need prescription refills, you’ll have periodic follow-ups for lab work and to make sure your dose is right. Some follow-ups may be available by telehealth or phone. There is a charge for follow-up visits.

  • Do you accept insurance?

We’re a concierge-style practice and aren’t contracted with insurance companies. We can provide a superbill you can submit to your insurance for possible reimbursement, and HSA/FSA funds are accepted.

  • Will Foothills replace my primary care provider?

No. We don’t provide traditional primary care services, and we encourage patients to keep a relationship with their Primary Care Provider.

  • Do you offer telehealth?

Yes. Telehealth visits are available for established patients across Idaho.
